Advance Care Planning

Planning for your future care (advance care planning) gives you the opportunity to think about, talk about and write down your wishes, preferences and priorities for your care. This includes how you would like to be cared for towards the end of your life. You can include anything that is important to you, no matter how insignificant it may appear.

Alzheimers Society

Provides support for those living with dementia, and run local groups.

Power of Attorney

This is a legal arrangement which gives someone else the right to make decisions if an individual is too unwell to do so (eg finances/social and health care). Forms can be downloaded via the link below or you can do this through a solicitor.

You can claim this if you need someone to help look after you (family/friend/carer).It is not means tested so you do not need to talk about your finances and it could be a great help financially

Crisis Response Team

Working with the East Midlands Ambulance Service, Northamptonshire Health NHS Foundation Trust, and Northamptonshire County Council; Olympus Care Services’ Crisis Response Team quickly responds to people who fall in their homes, preventing unnecessary hospital admissions and providing short-term care.
