Holiday vaccinations are a non NHS service. However we do offer two travel clinics per week for our patients who are travelling abroad.

If you are travelling abroad and think that you might need vaccinations – please make an appointment at our travel clinic.
These appointments can be booked by our receptionists on receipt of a completed travel form. You will be asked to check your blood pressure, height and weight when the appointment is booked.
The pack with the travel form can be downloaded below or it can be collected from our reception. The completed travel forms must be returned in person.
Please read through the leaflets as they contain important information relevant to your appointment and your travel.
The appointments for hoilday vaccinations must be AT LEAST 2 weeks prior to your travel dates. Most vaccines take 2 weeks to get into your system. The nurse will look at your travel form – if you do not need any vaccinations, she will phone you and cancel the appointment.
Most travel injections are free, but you may need to pay for some vaccines. Please bring adequate means of payment to your appointment.

We are now a centre for yellow fever vaccinations. You do not need to be a patient at Weavers Medical.
For patients at Weavers Medical, the nurse will advise you if you need a yellow fever vaccine.
If you are not a patient at Weavers Medical, ring the surgery on 01536 513494 and inform the receptionist that you need a yellow fever vaccination. A yellow fever travel form needs to be completed and returned to the surgery in person.
On receipt of the completed yellow fever travel form the receptionist will make you an appointment at our Travel Clinic. There is a charge of £55.00 for the Yellow Fever Vaccination (this charge is effective from 1st June 2013).
